Position Summary/Position


Under the direction of the Manager for Supportive Housing, within the LVN scope of practice, the incumbent is responsible for working directly with the IEHP Housing Team, contracted entities, and vendors to ensure coordinated, continuous cost-effective quality healthcare and community services for IEHP Members. The LVN for Supportive Housing will supply input into an interdisciplinary plan of care to sustain stable housing, thereby improving the Member’s social and physical health. The LVN for Supportive Housing is expected to use discretionary judgment based on their educational and professional training, when assessing Members for available housing and supportive programs according to established criteria.

Major Functions (Duties and Responsibilities)


1. Responsible for gathering of clinical documentation for housing referrals and assisting RN/SW for Housing, with on-going duties.
2. Works collaboratively with the Housing Team, to ensure referrals are addressed in a timely manner.
3. Serves as a resource for IEHP Members and collaborates with other internal departments, to assure Member has all wrap-around services to maintain Housing.
4. Assists with identifying potential cases for Case Management, Disease Management, Health Education and/or quality of care issues and making appropriate referrals when needed.
5. Responsible for timely and appropriate documentation in the Medical Management system.
6. Responsible for working with Team Members to support the goals of the department and the vision of the organization. 

Experience Qualifications


One or more years of experience working with the homeless population. Minimum three (3) years of experience in a clinical setting/health care delivery setting.

Education Qualifications


High school diploma or GED required.

Professional Licenses


Possession of an active, unrestricted, and unencumbered Vocational Nurse (LVN) license issued by the California BRN required.

Drivers License Required

Yes, must have a valid California Driver's license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points.

Knowledge Requirement


Knowledge of Community and Social Resources. Well versed in the use of motivational interviewing techniques.

Skills Requirement


Proficiency with Word, Excel and PowerPoint are highly desirable. Strong organization, presentation, written and oral communication skills.

Abilities Requirement


Ability to accurately document, and effectively engage with others with a collaborative manner.

Working Conditions


This position requires the staff to be off-site on occasion and meeting members in the community/facilities. 


Starting Salary: $57,948.80  - $73,860.80  

Pay rate will commensurate with experience
