Position Summary/Position


Under the direction of the Practice Coach Manager, the Practice Coach is part of a team that implements interventions at the practice level to improve clinical performance, targeted outcomes improving patient care, practice efficiency and achievement of practice transformation milestones. They are responsible for working with practices to facilitate redesign efforts, support care teams, introduce population health management and support achievement towards improved process, clinical outcomes and value-based care.  The goal of practice coaching is to progress toward a desired practice level transformational change. Practice Coaches have primary responsibility for practice coaching within external Enhanced Care Management (ECM) Provider (Model 1) sites.  They may also be deployed to other partner sites based on current strategic initiatives.  

Major Functions (Duties and Responsibilities)


1. Act as a liaison, teacher, facilitator and technical assistance provider for a subset of enrolled clinicians and their associated practices/agencies/facilities (practices) participating in Practice Transformation initiatives.
2. Provide on-site and remote technical assistance and practice facilitation to the clinicians, care teams and their associated practices per a set of intervention work plans to help practice improve quality measures and transformation in:
  a. Patient and family centered care design
  b. Continuous, data driven quality improvement
  c. Sustainable business operations
3. Facilitate trainings and practice guidance.  Technical assistance includes workflow design, implementing process improvement activities, EHR optimization, assessments, analysis, reporting and other work plan items.
4. Participate in coach workgroups and other learning sessions to share learnings and gain knowledge to better provide facilitation/teaching and technical assistance.
5. Participate in providing quality measures and completing practice assessments as required.
6. Maintain databases and work plans.
7. Actively participate as part of a team with IEHP management and consultants to conduct work and resolve any issues.
8. Establish and maintain a supportive role with Practice Transformation staff, network partner management, coaching teams, assigned clinicians, care teams and their associated practices.

Experience Qualifications


Minimum six (6) years of experience in a health-related area. Project management experience. Operations experience in a physician’s office or hospital. Experience with quality improvement. Experience with process improvement. Experience with EMR.

Preferred Experience


LEAN experience desirable. Experience with EMR. Experience with team-based care and population health management and tools preferred. Experience with complex care program design and implementation preferred.

Education Qualifications


Bachelor’s degree in Nursing from an accredited institution for licensed Registered Nurses.
Relevant Master’s degree (Nursing, Education, Social Work, Marriage & Family Therapist, Business Administration, Public Health, Organizational Development etc.) from an accredited institution required.

Professional Licenses


Possession of an active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN - for RNs with Bachelor’s degree.

Drivers License Required

Yes, must have a valid California Driver's License.

Knowledge Requirement


Knowledge and understanding of Medi-Cal population. Knowledge of concepts related to process and quality improvement in healthcare. Knowledge of clinical workflow development and/or redesign. Working knowledge of HEDIS and CAHPs. Knowledge of PCMH standards and recognition programs preferred.

Skills Requirement


Strong project management, communication and relationship management skills that can be used in a fluid program environment. Able to implement PDSA or like quality improvement methodologies using a data driven method. Highly organized and able to handle multiple priorities. Excellent oral and written communication skills. Excellent interpersonal skills, diplomacy, coaching and change management skills. Good Microsoft application skills (Word, Excel and PowerPoint).

Ability to work in an office environment with the flexibility to work remotely, potential travel throughout two counties. Starting Salary: $85,841.60  - $109,449.60
