Position Summary/Position
Social Determinants of Health, the Community Health Representative (CHR) is responsible for Representative is responsible for the implementation and coordination of initiatives that foster healthy communities through trusted collaborations that empower Inland Empire residents to live healthy and fulfilling lives. This is achieved by developing and maintaining strong and consistent relationships with IEHP Members, community-based organizations (CBO) and Providers. CHR routinely communicates organizational policies, procedures and processes, anticipate and responds to product/benefit educational needs and conducts ongoing CBO/Member/Provider and public entity trainings. CHR is a subject matter expert on strategies that promote diversity, equity and inclusion across systems of care. Their work is integral to the implementation of community, social and cultural strategic initiatives that lead to improved clinical quality metrics, service goals and compliance standards. The CHR is responsible for outreach to an assigned territory in the Inland Empire to market and educate on IEHP products by visiting CBOs, human services agencies, cultural centers and public entity partners in settings such as health fairs, senior/community centers, professional training facilities, and providing marketing materials to be distributed to potential members. This position will be assigned a Medi-Cal marketing territory for the purpose of outreach and education.
Major Functions (Duties and Responsibilities)
1. The Community Health Representative is responsible for coordinating/accomplishing activities, including:
a. Contribute to the development of programmatic goals, objectives, policies, and procedures; assists in program development and tracking.
b. Identify programmatic challenges and assists in designing or revising program operations and procedures to improve efficiencies.
c. Market and educate on IEHP products at community events, health fairs, and provider health events, including staffing, and maintaining IEHP booths.
d. Develop and maintain relationships with IEHP Providers, County employees, schools, community-based organizations, and disability and minority community leaders.
e. Conduct formal and informal training on IEHP benefits, features, and managed care concepts to State contracted Medi-Cal enrollees, County employees, IEHP Provider staffs, and community program staff.
f. Serve as a liaison to major Department of Social Services Offices, Area Agencies on Aging, Social Security Offices, Hospitals, and other organizations where potential synergies exist.
g. Plan and coordinate meetings and events for the Inland Empire Disabilities Collaborative (IEDC).
(i) Plan and coordinate monthly IEDC meetings including, but not limited to, programming, marketing, networking, and logistics.
(ii) Plan and coordinate the IEDC Expo and Conference including, but not limited to, logistics, fundraising, attendance, budgeting, marketing, entertainment, audiovisual, and exhibitor development.
h. Plan, coordinate, and conduct internal trainings including, but not limited to; Disability Sensitivity, Independent Living Philosophy, and specific subject matter trainings as requested by IEHP Team Members in order to improve communication, support and accessibility for minority groups including seniors and persons with disabilities.
i. Plan and coordinate diversity awareness events and activities for IEHP Team Members and Providers to train and inform about topics relating to diversity, equity and inclusion such as cultural humility, sex and gender minorities, people with disabilities, seniors, individuals with limited English proficiency and populations experiencing health disparities.

Experience Qualifications
Two (2) years social, community, communication, health plan or market/sales services related work experience required.
Preferred Experience
Education Qualifications
Bachelor’s degree in Human Development, Public Health, Sociology, or any related field from an accredited institution required.
Preferred Education
Professional Certification
Must be able to satisfactorily pass IEHP’s Medi-Cal Marketing test within three (3) months of employment.

Knowledge Requirement
Progressive understanding of Medicare and Medi-Cal and related health insurance options. Knowledge of Medicare managed care and Medi-Cal managed care preferred. Understanding and sensitivity to a multi-cultural environment and community.
Skills Requirement
Bilingual (Spanish/English/Chinese/Vietnamese) preferred.
Verbal and written, word processing and spreadsheet capabilities. Data input and basic analysis. Excellent interpersonal/communication skills, public speaking, and presentations skills with communities of diverse education, socio-economic, cultural and professional backgrounds.

Working Conditions
Must be willing to work evenings and weekends. Must be willing to travel.
Community Health Representatives will have on-going, face-to-face and group connections, conversations, and presentations with IEHP members, general community, community/faith-based agencies, other partnering agencies and leaders.

Physical Requirements
A reasonable salary expectation is between $53,892.80 and $68,702.40, based upon experience and internal equity.
