Position Summary/Position

 

Under the general direction of the TOC Manager, the CBAS RN is responsible for working directly with Independent Practice Associations (IPAs), medical groups, Providers, community agencies, Members/caregivers, and other entities supporting Member care to ensure coordinated, continuous care and serve as a resource person for these facilities. 

Major Functions (Duties and Responsibilities)

 

1. Conduct face-to-face nursing assessments for potential CBAS Members utilizing the CBAS Eligibility Determination Tool (CEDT).
2. Ensure the health plan has a copy of the CBAS Individual Plan of Care (IPC) in the medical management system (mms).
3. Conduct Member Outreach calls to educate potential Members regarding CBAS services.
4. Facilitate referrals to appropriate CBAS Centers.
5. Schedule face-to-face assessments, complete CEDT, and submit to the Medical Director for review and approval. 
6. Act as a Member Advocate.
7. Maintain positive working relationship with CBAS center staff.
8. Assist with coordination of care as needed. 
9. Work collaboratively with Care Management to ensure Members/caregivers are meeting IPC goals.
10. Assist Members with immediate needs with regard to access, referrals and authorizations.
11. Assist Member with resources such as Long-Term Support Services (LTSS), Disabilities, Behavioral Health, and other community resources.
12. Comply with IEHP Policies with traveling to and from CBAS Centers.

Experience Qualifications

 

Three (3) or more years of care management experience in a health care delivery setting.  

Preferred Experience

 

Minimum one year clinical experience in an acute care facility, skilled nursing facility, home health or clinic setting preferred.

Experience in an HMO or experience in Managed Care setting preferred.

Education Qualifications

 

High school diploma or GED required.

Professional Certification

Professional Licenses

 

Possession of an Active, unrestricted, and unencumbered Registered Nurse (RN) license issued by the California BRN.

Drivers License Required

Yes, must have a valid California Driver's License.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Possession of a valid State of California driver’s license and active automobile insurance coverage.

 

Starting Salary: $75,296.00  - $95,992.00  

Pay rate will commensurate with experience
