Position Summary/Position


The Community & Housing Navigator is responsible for providing social service linkage to IEHP Members needing community, county, health, and social services including Long-Term Services and Supports (LTSS) and services for Members at risk of or experiencing homelessness. The Community & Housing Navigator will focus on essential resources that allow a Member to reside in the community in appropriate housing and in the least restrictive environment possible.  The Community & Housing Navigator will collaborate with IEHP Clinical teams, LTSS team, housing team, Intensive Case Management Service providers and Coordinated Entry System staff to successfully transition Members from a state of homelessness into permanent supportive housing.  The Community & Housing Navigator will provide training and coaching to program stakeholders on established programmatic process, philosophy, and community resource linkage.  The applicant must be a proactive team player who is also able to work independently in assigned communities, and build rapport with diverse Members, providers, and local partner agencies.

Major Functions (Duties and Responsibilities)


1. Process incoming LTSS or housing program referrals and prepare them for review and submission to the appropriate program including LTSS or contracted Integrated Case Management Services (ICMS) providesr.
2. Collaborates closely with the referral source to ensure Members at risk of homelessness or at risk of placement in a long-term care facility are appropriately matched with interventions that will most effectively end their homelessness or prevent, divert, or delay placement.
3. Collaborates closely with Providers, community-based organizations, county agencies, and IEHP contracted vendors to coordinate linkage. 
4. Provides information, referrals, linkages, and advocacy to assist Members in accessing services and resources telephonically or in person in the field.
5. Serve as resources and referral team lead for LTSS and housing resources.
6. Collaborates with reporting units to generate project tracking reports and visual management tools.

Experience Qualifications


Two (2) years of experience in coordinating social services for members in need. Training in working with low-income families, people experiencing homelessness, and seniors and people with disabilities. Community event participation. Delivering presentations/trainings to external and internal partners.

Preferred Experience

Education Qualifications


High school diploma or GED required.

Preferred Education


Associate’s degree in Social Work or related field from an accredited institution preferred.

Professional Certification


Medical Assistant or related field experience preferred.

Professional Licenses

Drivers License Required

Yes, must have a valid California Driver's License.

Knowledge Requirement


Knowledge of Medi-Cal/managed care environment preferred. Understanding of and sensitivity to multi-cultural communities. Knowledge of Harm Reduction/Housing First Model and commitment to that model. Knowledge of LTSS and community-based resources in the Inland Empire. Managed Care knowledge.

Skills Requirement

Working Conditions


Word processing and data involving computer keyboard, viewing monitor. Communication with IEHP members and staff, which includes intensive telephone use. Must be willing to work evenings and weekends as needed.

Starting Salary: $48,110.40  - $59,425.60  

Pay rate will commensurate with experience
