Position Summary/Position

 

Under the general direction of the Care Management Manager, the Care Manager, MSW is responsible for working directly with the Plan, IPA medical groups, providers, hospitals, community agencies, Members/caregivers, and other entities supporting Member care to ensure coordinated, continuous care and serve as a resource person to these facilities.

Major Functions (Duties and Responsibilities)

 

1. Responsible for serving as a resource for IEHP Members and Providers for care management and continuity of care.
2. Responsible for assessing Members for appropriate care management needs:
    a. Develop, implement, and evaluate individualized care plans (ICP)
    b. Act as a Member advocate
    c. Serve as a liaison between Member, Caregivers, and Providers
    d. Educate and support Members Caregivers with meeting ICP goals
    e. Collaborate with County agencies for enhanced Member experiences
3. Assist with referrals for IEHP Members that are eligible for carve out programs such as CCS, multi-organ transplant, etc.
4. Assist members with immediate needs with regard to access, referrals and authorizations.
5. Assist member with resources such as LTSS, Disabilities, Behavioral Health, and community resources.
6. Responsible for any other duties as required to ensure successful care management processes and Member outcomes.   

Experience Qualifications

 

Three (3) years experience coordinating care/services for seniors or persons with disabilities.

Education Qualifications

 

Master of Social Work degree from an accredited institution required.

Professional Certification

Drivers License Required

Yes, must have a valid California Driver's License.

Knowledge Requirement

 

Knowledge of community resources and health plan benefits.

Skills Requirement

 

Skillful at offering information and support to Members and their families/caregivers. Research community resources. Skillset of computer applications, including word processing, database and spreadsheet is required.

Abilities Requirement

 

Ability to interview, assess, and coordinate care. Requires ability to communicate and work effectively with a variety of providers and maintain positive relationships with internal and external contact at all levels. Must be able to work independently.

Starting Salary: $75,296.00  - $95,992.00  

Pay rate will commensurate with experience
